Lines that run vertically down one cheek or across the forehead at an odd angle often do not match any expression. Their orientation follows what the face rests against overnight.

How compression differs from expression

Expression lines form perpendicular to the pull of the muscle beneath them, which is why forehead lines run horizontally and frown lines run vertically between the brows.

Sleep lines follow the direction in which skin is folded against a surface. They frequently run diagonally and appear on one side only.

Dermatologists use that asymmetry and orientation as the distinguishing feature, since it does not correspond to any muscle's action.

Why duration matters more than force

The pressure involved is modest. What differs from a brief fold is that it is maintained for hours without release, night after night.

Skin recovers from short deformation through its elastic fibers. Prolonged and repeated folding gives those fibers less opportunity to return fully to their prior state.

Recovery capacity also declines as the dermal structure changes with age, which is why sleep creases that once faded within an hour begin persisting into the day.

What happens in the dermis

The dermis contains collagen for tensile strength and elastin for recoil, arranged in a network that resists and then reverses deformation.

Elastin is produced mainly early in life and is not replaced at a meaningful rate afterward, so damaged elastic fibers accumulate rather than being renewed.

Repeated mechanical stress along a fixed line concentrates that accumulation, which is why the crease appears in the same place each time.

What surface and position change

Sleeping on the back removes facial contact with the pillow entirely, which is the only approach that addresses the mechanism directly.

Smoother pillow surfaces reduce friction and allow skin to slide rather than bunch, which lessens folding without changing the pressure.

Neither approach reverses lines that already exist, and claims that a pillowcase removes wrinkles describe an outcome the material cannot produce.

Why this is a small factor overall

Cumulative ultraviolet exposure is the dominant driver of visible skin aging, followed by intrinsic changes in the dermis over time.

Sleep position is a contributing mechanical factor and not a primary cause, which is worth keeping in proportion before rearranging a sleep routine around it.

Lines that appeared suddenly, or asymmetry accompanied by drooping or weakness, are medical findings and belong with a physician rather than a skincare decision.
